Application Process
Ensure the applicant meets the admissions requirements listed below. To help SA Life Academy learn more about your adult, please click here to fill out a prospective family questionnaire.
Once the questionnaire is completed, you will receive an email from samantha@salifeacademy.org to set up a tour of the campus. Your adult needs to be present at the tour so our staff can get to know him/her better.
Fill out the application you receive at the tour and return it to SA Life Academy or email it to Samantha at samantha@salifeacademy.org.
Once an application is received, you will be added to our waitlist.
Admission Requirements
SA Life Academy is designed to provide a safe, supportive, and community-based program for adults with intellectual and/or developmental disabilities. To be considered for admission, applicants must meet the essential requirements of the program, including:
Be at least 18 years of age;
Have an intellectual and/or developmental disability;
Be able to participate in a group-based program without requiring continuous one-on-one staff support;
Be able to transition between activities with the level of support provided by SA Life Academy;
Be able to manage toileting and personal hygiene independently or with limited assistance that does not require continuous one-on-one staffing or ongoing attendant-level personal care;
Be able to participate safely in reasonably crowded, stimulating, and community-based environments;
Not require medical care or nursing services that SA Life Academy is not equipped or staffed to provide, as SALA does not have an on-site nurse or medical staff; and
Meet SA Life Academy's applicable behavioral and safety expectations.
Admission decisions are made on an individualized basis and take into consideration the applicant's needs, abilities, safety, and the supports available within the SA Life Academy program.
